Clovis, California

Clovis, the "Gateway to the Sierra", is located in the midst of the agriculturally rich San Joaquin Valley. The sprawling "western" style city boasts a population exceeding 90,000 and encompasses 23 square miles. The city boasts an amazing 26 miles of trails that wind throughout it. Detailed Benefits Information

Medical and Dental Benefits

  • FlexChoice - Full-time or part-time (more than 40 hours worked per pay period) employees are eligible to choose from the benefit options listed below. Employees are eligible to receive medical/dental insurance coverage on the first day of the month following 30 days of employment.
  • Medical Plans - Employees can choose from three medical plans, including an HMO and two PPO options. All three plans include automatic vision care coverage, and coverage in Community's prescription drug plan.
  • Dental Plans - Community offers two dental plans: a traditional plan/Preferred Provider Organization (PPO) and Dental Health Maintenance Organization (DHMO) dental plan.
  • Health Reimbursement Account - Employees can receive reimbursement for eligible medical, dental and vision care expenses (for themselves or a family member) not paid by any additional health coverage they may have. This is done on a pre-tax basis.
  • Dependent Care Reimbursement Account - Employees can set up a reimbursement account for childcare expenses they incur. This is done on a pre-tax basis.
  • Long-Term Disability - Community's basic long-term disability plan provides 50% of an employee's base pay. A "buy-up" plan provides 66.6%, or 2/3, of an employee's base pay.
  • Optional Term Life Insurance - Employees have the option to purchase additional term life insurance at a reasonable cost for themselves and their family

Other Insurance

  • Basic Life Insurance Accidental Death & Dismemberment - Full-time and part-time (working 40 or more hours per pay period) employees are eligible for basic life insurance and accidental death and dismemberment coverage. Basic life insurance coverage for non-exempt employees is $10,000 and two times the annual salary for exempt employees. Basic death and dismemberment coverage is the same.
  • Workers' Compensation- All Community employees are eligible for hospital, physician care and compensation for job-incurred accidents or illness as governed by state law.
  • Unemployment Insurance- Upon termination, all Community employees may apply for state unemployment assistance. State law governs eligibility and the amount and number of payments.
  • Professional Liability Insurance - All Community employees are eligible for professional liability insurance provided by Community. The insurance provides coverage for performance of assigned duties as carried out within the scope of the job description and hospital policy.
  • State Disability Insurance (short-term)- All California employees are covered for up to 52 weeks for non-industrial illness or injury.

Time Off

  • Paid Time Off (Vacation, Holiday, Sick)
  • Full-time staff: Employees can accrue up to 19 holiday/vacation days, 6 discretionary and 6 extended sick leave days, for a total of 31 days for service up to 5 years. For 5 to 9 years, 36 days per year. For 10 years or more, 41 days per year.
  • Part-time staff: Employees can accrue up to 19 holiday/vacation days for hours equivalent to 4 years of service, up to 24 days for 5 to 9 years, and up to 29 days for hours of service equivalent to 10 or more years. Actual accrual is based on hours worked.
  • Medical Leave of Absence - Full-time and part-time employees are allowed medical leave of absence without pay.
  • Personal or Educational Leave of Absence - Full-time and part-time employees are allowed personal or educational leave of absence without pay.
  • Jury Duty - Full-time employees receive their base salary equivalent for a maximum of 30 days while serving as a juror.
  • Bereavement Leave- Full-time and part-time (working at least 40 hours per pay period) employees may take up to three days of leave with full pay in case of death in the immediate family. One day of leave with full pay is granted in case of death of grandparents, grandchildren, mother-in-law or father-in-law.
  • Maternity Leave - All Community employees are eligible to receive up to four months of leave for medically certified disability for pregnancy or childbirth.
  • Family Leave - Full-time and part-time (working at least 40 hours per pay period) employees are eligible for up to 12 weeks of leave coordinated with paid leave (if available) for personal illness, birth or adoption of a child, serious illness of a child, or to care for a parent or spouse who has a serious health condition.
  • Military Leave of Absence - Full-time and part-time employees are granted leave without pay to serve in the military.

Retirement

  • Retirement Plan - After working 1,000 hours in 12 months immediately preceding semi-annual enrollment, all Community employees are eligible to enroll in Community's retirement plan. Upon retirement, employees have the money they contributed, the money Community contributed and the interest earned (vested after five years). Employees also may take their account balance with them if they leave Community before retiring.
  • Supplemental Savings Programs - All employees are eligible to participate in a Tax Sheltered Annuity Plan (TSA) immediately upon hire. For 2006, employees can contribute up to $15,000 per year, pre-tax. There are catch-up provisions for those who are 50 years or older or have at least 15 years of continuous service with Community.
